The following items are requested in order to calculate an accurate quote. Please allow adequate time to provide a formal proposal in advance of the current plan’s renewal.

• Renewal date of the current plan
• Current plan of benefits
• Census including:
• Total number of males and females
• Dates of birth
• Type of coverage
• Number of single / employee +1 / family coverages
• 2 years of claims experience (by month if possible)
• List of employees with claims greater than $25,000 with the diagnosis and the prognosis
• Name of your present broker or consultant

Experience: The Miller-Lewis Benefit Consultant staff consists of professionals with in depth experience. For our employers, we provide complete oversight of benefit plans directly assisting plan administrators as well as employees. We have been the advisor for the South Central Ohio Insurance Consortium (SCOIC) since it’s start in 1982. Our life, accident and small group agent has over 30 years experience in the insurance industry. Our extensive understanding of available policy types will assist in finding the right health coverage to get the most for your healthcare dollar.
Demonstrated Growth: Miller-Lewis was established in 1982 by Kent A. Miller and John C. Lewis specializing in group health and personal insurance. Kent became the sole owner of the business upon the John’s retirement in 2005. Our growth has demonstrated the success in providing employee benefits for both the private and public sector.
Independent: Miller-Lewis Benefit Consultants acts as an independent consultant without association with any insurance company, third-party administrator or provider network.
Prompt Service: Our professionals will give precedence and will act immediately to all requests that have a medical ur-gency. Most emails and phone requests are responded to the same day we receive them. If we are unable to immediately respond to your request, we work to respond to most issues within 24 hours.

In 1996 our agency formed the South Central Ohio Insurance Consortium (SCOIC) for public entities and school districts in central Ohio. SCOIC provides health care and related insurance benefits to 12 member organizations in 4 Ohio counties. This represents benefit coverage for more than 2,700
employees and their dependents.
Ohio Housing Authority Commission (OHAC)

We appreciate the importance of all confidential information and are committed to protecting it as we provide for your Consortium’s needs.
We do not sell, trade or otherwise disclose your identity or any other personal information to third parties for their marketing purposes. This includes information we obtain concerning all applicants, present and former members.
While your nonpublic information is under our control, access is limited to our employees who have a legitimate business need for such information with respect to your insurance coverage. In accordance with applicable state and federal regulations, we maintain physical, procedural and electronic safeguards to protect the privacy and confidentiality of your nonpublic personal information.
Technical qualifications are in place for incorporation with our partner members within our Consortium’s and TPA’s. They allow us to provide seamless and secure services directly to employees, member groups and board members. E-mails with sensitive information are sent secure. We use an internal management system (IMS) program to enter and document services and files with two separate off site back up locations, maintain HIPAA compliant operations, and utilize electronic scanning to log all documents.
